Students Requiring Special Diets

Students who require modifications to their school meals for a diagnosed medical condition (ex. allergy, diabetes, celiac disease, etc...) must have a completed Diet Order on file at the 草榴社区 Nutrition Office. These requests must be completed on the Medical Statement for Students with Special Nutritional Needs for 草榴社区 Meals (Diet Order) form, signed by a Recognized Medical Authority. Diet Order forms may be obtained from the 草榴社区 Nutrition website, 草榴社区 Nutrition Office, Cafeteria Manager, or 草榴社区 Nurse. These forms should be returned to the 草榴社区 Nurse. The Nutritionist will modify menus and instruct Cafeteria Managers with regard to each individual Diet Order.
